Wellness Recovery Action Plan (WRAP)

WRAP, or the Wellness Recovery Action Plan, is a personalized wellness tool that empowers individuals to identify their strengths, manage mental health challenges, and take control of their recovery. Through community outreach and mental health peer support, participants can create self-designed plans that incorporate housing support and recovery coaching. This approach helps them learn to maintain wellness, recognize triggers, and develop strategies to stay on track—even during tough times—fostering a holistic approach through Whole Health Action Management.

Whole Health Action Management (WHAM)

WHAM is a peer-led program that empowers individuals to take charge of their physical and mental well-being through effective Mental Health Peer Support. It emphasizes setting personal health goals, building daily habits, and utilizing wellness tools, including the Wellness Recovery Action Plan, to enhance quality of life. Participants engage in Community Outreach to foster connections and collaborate in managing stress, improving sleep, eating well, staying active, and creating balance in their lives, all while receiving Recovery Coaching and Housing Support as needed.

Mental Health Peer Support

Peer support connects individuals with trained mentors who have lived experience with mental health challenges and recovery. These supportive relationships foster trust, empathy, and hope—offering guidance from someone who truly understands the journey. Through community outreach, peer supporters provide essential mental health peer support, walking alongside participants to help them set goals and access resources, including housing support. They encourage individuals throughout the recovery process, often utilizing tools like the Wellness Recovery Action Plan and Whole Health Action Management to enhance overall well-being.

Recovery Coaching

Recovery Coaching is a strengths-based, non-clinical support service that plays a vital role in community outreach by helping individuals set and achieve personal goals related to their mental health, wellness, and life purpose. Coaches provide essential mental health peer support, offering encouragement, accountability, and practical strategies to assist participants in building confidence, making healthy choices, and remaining committed to their recovery journey. Additionally, elements like the Wellness Recovery Action Plan and Whole Health Action Management are integrated to ensure comprehensive support. It’s all about moving forward—with support every step of the way.

Community Outreach

Our Community Outreach efforts bring mental health education, resources, and support directly into the neighborhoods we serve. Through events, partnerships, and local engagement, we focus on providing Mental Health Peer Support and Housing Support to break stigma, build trust, and connect individuals and families to the help they need. Housing Support

Stable housing is crucial for long-term recovery, and at Bloom Recovery Inc., we emphasize the importance of community outreach to provide effective housing support. Our dedicated team offers assistance in navigating housing resources, applications, and transitional living options tailored for individuals confronting mental health challenges. Through mental health peer support, we connect you with local shelters, identify supportive housing programs, and help you create a Wellness Recovery Action Plan for independent living. With our recovery coaching, we walk alongside you every step of the way toward achieving safe and stable housing through whole health action management.

Forensic Peer Mentoring

Forensic Peer Mentoring

Forensic Mentoring offers supportive guidance to individuals involved in the justice system who are also navigating mental health or substance use challenges. Our mentors provide lived experience, advocacy, and resources to help participants transition successfully and rebuild their lives. This program fosters accountability, healing, and long-term recovery through consistent, trusted relationships. Our FPM’s work hard to change the narrative for returning citizens and reduce recidivism rates.
